Association between the pharmacological treatment of bipolar disorder and risk of traumatic injuries: a self-controlled case series study

Background
Patients with bipolar disorder (BPD) are prone to engage in risk-taking behaviours and self-harm, contributing to higher risk of traumatic injuries requiring medical attention at the emergency room (ER).We hypothesize that pharmacological treatment of BPD could reduce the risk of traumatic injuries by alleviating symptoms but evidence remains unclear. This study aimed to examine the association between pharmacological treatment and the risk of ER admissions due to traumatic injuries.
MethodsIndividuals with BPD who received mood stabilizers and/or antipsychotics were identified using a population-based electronic healthcare records database in Hong Kong (2001–2019). A self-controlled case series design was applied to control for time-invariant confounders.
ResultsA total of 5040 out of 14 021 adults with BPD who received pharmacological treatment and had incident ER admissions due to traumatic injuries from 2001 to 2019 were included. An increased risk of traumatic injuries was found 30 days before treatment [incidence rate ratio (IRR) 4.44 (3.71–5.31), p < 0.0001]. After treatment initiation, the risk remained increased with a smaller magnitude, before returning to baseline [IRR 0.97 (0.88–1.06), p = 0.50] during maintenance treatment. The direct comparison of the risk during treatment to that before and after treatment showed a significant decrease. After treatment cessation, the risk was increased [IRR 1.34 (1.09–1.66), p = 0.006].
ConclusionsThis study supports the hypothesis that pharmacological treatment of BPD was associated with a lower risk of ER admissions due to traumatic injuries but an increased risk after treatment cessation. Close monitoring of symptoms relapse is recommended to clinicians and patients if treatment cessation is warranted.

In interpreting radiocarbon dating results, it is important that archaeologists distinguish uncertainties derived from random errors and those from systematic errors, because the two must be dealt with in different ways. One of the problems that archaeologists face in practice, however, is that when receiving dating results from laboratories, they are rarely able to critically assess whether differences between multiple 14C dates of materials are caused by random or systematic errors. In this study, blind tests were carried out to check four possible sources of errors in dating results: repeatability of results generated under identical field and laboratory conditions, differences in results generated from the same sample given to the same laboratory submitted at different times, interlaboratory differences of results generated from the same sample, and differences in the results generated between inner and outer rings of wood. Five charred wood samples, collected from the Namgye settlement and Hongreyonbong fortress, South Korea, were divided into 80 subsamples and submitted to five internationally recognized 14C laboratories on a blind basis twice within a 2-month interval. The results are generally in good statistical accordance and present acceptable errors at an archaeological scale. However, one laboratory showed a statistically significant variance in ages between batches for all samples and sites. Calculation of the Bayesian partial posterior predictive p value and chi-squared tests rejected the null hypothesis that the errors randomly occurred, although the source of the error is not specifically known. Our experiment suggests that it is necessary for users of 14C dating to establish an organized strategy for dating sites before submitting samples to laboratories in order to avoid possible systematic errors.

Aims and method
To illustrate the clinical benefit of polygraph testing for mentally disordered sex offenders at a high secure psychiatric hospital. It is a retrospective review of two patients' case notes and of interviews with clinicians. Post Conviction Sex Offender Testing (PSCOT) was used to assist these patients in making disclosures regarding their sexual history and to aid their treatment.
ResultsPost Conviction Sex Offender Testing was responsible for bringing about new disclosures relating to the patients' sexual histories and aiding their treatment progression to lower secure settings. New information was incorporated into the patients' treatment programmes and risk management plans. Post Conviction Sex Offender Testing also encouraged a more honest and effective participation in sex offender treatment programmes and allowed the evaluation of antilibidinal medication.
Clinical implicationsPost Conviction Sex Offender Testing should be more widely considered among mentally disordered sex offenders who have been challenging to treat, as it has the potential to aid their management and progression to lower-security settings.

We developed the Hong Kong Specific Learning Difficulties Behavior Checklist for Junior Secondary School Students (BCL-JS) for teachers to rate the frequency of 52 reading-related behavioral characteristics of Chinese secondary school students. An item factor analysis based on ratings on 947 students yielded seven distinct dimensions. In a separate sample of 90 students, the seven constructs of the BCL-JS significantly correlated with students’ performances on most literacy and reading-related cognitive measures, and differentiated adolescents with or without dyslexia. Discriminant analysis showed that the BCL-JS had a high rate of correct classification (82.2%). These findings support that the BCL-JS is a reliable screening tool for Chinese junior secondary school students at risk for dyslexia.

The present study examined word learning difficulties in Chinese dyslexic children, readers of a nonalphabetic script. A total of 105 Hong Kong Chinese children were recruited and divided into three groups: Dyslexic (mean age 8;8), CA control (mean age 8;9), and RL control (mean age 6;11). They were given a word learning task and a familiar word writing task. It was found that the Dyslexic group performed less well than the RL group in learning irregular words over trials but not the regular ones. Error analyses showed that the Dyslexic group made more orthographic and word association errors but less intra-wordlist interference errors than the RL control group. The Dyslexic group also performed significantly less well than both control groups in writing familiar words (e.g. their own name). These findings suggest that Chinese dyslexic children have difficulty learning new words, especially irregular ones, and retaining overlearned words in long-term memory. We conclude that Chinese dyslexic children have a specific impairment in word learning like their alphabetic counterparts.
Bottom-Gate TFTs with Channel Layer Grown by Pulsed PECVD Technique

In this paper, we report on nc-Si:H thin films deposited by the pulsed PECVD technique at a temperature of 150C and TFTs made using this material. RF power and silane flow rate were varied in order to study the effect of different levels of crystallinity on the film. Electrical conductivity, Hall mobility, optical transmittance, and Raman backscattering were measured on films of two different thicknesses. From the Raman data we see that the 50 nm films with hydrogen dilution are mostly amorphous, indicating the presence of a thick incubation layer. The values obtained for the conductivity, mobility, and optical gap varied depending on the processing conditions and these results are discussed. Bottom-gate TFTs were fabricated using a pulsed PECVD channel layer and a SiN gate dielectric. The TFTs' extracted parameters are msat μsat ≤ 0.38 cm2=(V s), Vt,sat ≥7:3 V, Ion/off >106, and S < 1 V/decade. The TFT performance and material properties are presented and discussed.

Introduction
The importance of growth hormone (GH) deficiency (GHD) in adulthood as a disease entity first became apparent in the late 1980s when clinical studies, facilitated by the ready availability of recombinant human GH (rhGH), demonstrated the beneficial effects of GH replacement in GH-deficient adults (Jørgensen, et al., 1989; Salomon et al., 1989). It has subsequently been shown that GH-deficient adults have several abnormal clinical features including increased adiposity (Hoffman et al., 1995), reduced muscle strength and exercise capacity (Cuneo et al., 1990; Rutherford et al., 1991), reduced sweating (Juul et al., 1993) and impaired psychological well-being (McGauley et al., 1990; Rosén et al., 1994). Although such clinical characteristics have become recognized as a clinical syndrome of adult GHD (see Chapter 8), the syndrome and its components are not sufficiently distinctive to enable a confident diagnosis. Some of these features are a normal accompaniment of ageing, in particular the reduction in lean tissue mass and increase in fat mass (Forbes, 1987). In view of the non-specific clinical features of adult GHD specific biochemical criteria remain necessary for diagnosis.
There are three conventional approaches to the evaluation of GHD. These are measurement of: (i) peak GH response to provocative stimuli; (ii) measurement of spontaneous GH secretion derived from repetitive sampling over extended periods; and (iii) biological markers of integrated GH action including serum insulin-like growth factor-I (IGF-I) and IGF binding protein-3 (IGFBP-3). An evaluation of the extensive paediatric experience demonstrates poor concordance between the various tests (Bercu et al., 1986; Rose et al., 1988; Lee et al., 1990) and this has resulted in a long standing controversy as to which is the most accurate test for the diagnosis of GHD.
